I have not yet been diagnosed with ADHD/ADD, but the way you describe how games like GTAV really resonates with me. It also explains why I loved most final fantasy games as they offered the right balance of exploration and story progression. Even once you hit the 'world map' you often would only have a few limited places to go to which offered much better direction.
